Volunteer Application and consent forms


  • Volunteers that will provide childcare, must complete and submit the REFERENCE FORM prior to your assignment.
  • All volunteers must be at least 15 years old at the time of the event. 
  • If the volunteer is  under 18 years old, there must be a PARENT/GUARDIAN CONSENT FORM  completed and signed prior to or the day of the event.
