Volunteer Application and consent forms

Volunteers that will provide childcare, must complete and submit the REFERENCE FORM prior to your assignment.
All volunteers must be at least 15 years old at the time of the event.
If the volunteer is under 18 years old, there must be a PARENT/GUARDIAN CONSENT FORM completed and signed prior to or the day of the event.
